Find a primary care provider

Look on your Member ID to find your current provider. If you would like to select a new provider, you can search our directory and call Customer Service to request a change.

Your primary care provider relationship

Un proveedor de atención primaria, comúnmente conocido como PCP, es quien mejor conoce su estado de salud. Su PCP podría ser un médico, un enfermero especializado o un asistente médico. If needed, your PCP can refer you to a specialist for more specific care. CareOregon cubre todos estos servicios. Start with your PCP for all your health care needs.

It's important to have a trusting relationship with all your providers-including physical, dental and mental health providers. A close relationship means your provider knows you well, and you feel comfortable when you see them. Health Share/CareOregon wants to help you build that relationship and find a provider who works best for you.

Wellness visit

As a CareOregon member, you get assigned to a PCP, and you'll find their name and number on your Member ID card. Pero también es importante que su PCP lo conozca a usted.

As soon as you get your Member ID card, make an appointment with your PCP for a regular check-up, or wellness visit, even if you’re not sick.

Es bueno que se prepare para aprovechar al máximo la visita a su PCP. Lleve una lista de preguntas que pueda tener. Also, bring a list of all the medications you're taking.

¿PCP, atención de urgencia o sala de emergencias?

Go to your PCP for:

  • Chequeos
  • Tests and health screening
  • Mild illness (cold, flu, constipation, allergic reactions, sprains, etc.)
  • After-hours care (call to see if available)

Go to urgent care if:

  • You need immediate care and your PCP tells you to go to urgent care or can’t see you
  • You aren’t assigned to a PCP

Go to the emergency room for life-threatening symptoms, like these:

  • Dolor de pecho
  • Dificultad para respirar
  • Traumatismo craneal
  • Angustia mental

If you believe your health is in serious danger, go to the nearest hospital or call 911 for help.
